Lively atmosphere On-the-job training Growth opportunities Safe work environment Relaxed atmosphere Qualifications Candidates must have strong communication skills, experience with EMR, multiphone lines and at least 1 year of medical office experience Must be able to call and communicate well with patients over phone Must also have basic knowledge of chronic medical conditions such as hypertension and diabetes Must be able to Multitask Cardiology Responsibilities Working closely with our Care Manager, the CCM assistant will provide outreach calls to patients enrolled in our Chronic Care Management Program The MA will be responsible for assessing chronic conditions via phone, medication reconciliation, reviewing and updating care plans under the direction of the physician and coordinating with the care team to help patients meet their goals In addition, the MA will meet regularly with the care team and providers to review care plans for patients and discuss any potential clinical concerns identified The MA will be expected to reach out to all assigned patients daily and to document all calls in the EHR to ensure compliance with the CCM program You will need to be able to call in prescriptions and make appointments This is a full-time position working Monday through Friday Job Type:

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
